Please enter the driver's license number of the current vehicle owner. If there is more than one owner with "and" separating their names on the title, please enter all owner's driver's licenses numbers. "Or" titles only require one owner's information

After your lien release, we’ll need some more information, so keep going until the form is complete.

If you have a lien on your vehicle, the title is held electronically by the state. You can still add a transfer on death to your title if you obtain consent from your lienholder to do so. Please reach out to your lienholder or bank to get this consent form.
